We believe in providing a home that belongs to our residents. We have a person-centred care philosophy that influences everything we do.
We promote shared decision-making, resident choices and flexibility in day-to-day living. We believe in supporting our residents right to self-determination and independence, and include them in planning and implementing our programs.
We acknowledge and respect that families are an active part of our team and we encourage their involvement through our Care Partner Program.
We strive to assign the same group of staff to our residents. This consistency helps decrease anxiety and confusion and encourages our residents to build relationships. This also allows our staff to learn about the residents and provide each person with personalized care.
Before a resident moves into Loch Lomond Villa, we conduct a full assessment of their needs, abilities and wishes. We gather information from the resident, their family and our partners. Following the assessment, we put together a personalized care plan for each resident
Our residents are encouraged to participate in the decisions regarding their care and their decisions are respected. When mutually agreed upon and clinically appropriate, we encourage families to participate in the emotional, spiritual and physical care and support of the resident.
Loch Lomond Villa offers various levels of nursing care to those requiring assistance. Our professional employees provide care, rehabilitation and special programs, with a commitment to guiding our residents toward social, spiritual, psychological and physical well-being.
Through this commitment, we have also created special environments for helping residents with dementia and those who are mentally frail. The person-centered care we deliver is tailored to meet the unique needs of each resident. We have private and semi-private rooms available, furnished with special mementos and personal belongings – before long, its home.
Community partners are very important to Loch Lomond Villa, as we are committed to the pursuit of excellence and continuous improvement in the delivery of holistic care. We will continue to strengthen links with the community and offer appropriate access to services based on the community needs and available resources.
Loch Lomond Villa has adopted a Performance Excellence Process that is a results-oriented, long-term approach to continuous improvement in person-centered care. The approach focuses on four main pillars:
This process optimizes structure and work environment, enhances communication, empowers staff and produces more cohesive teams. Education and training are encouraged, cooperation, teamwork and partnering is recognized, and everyone contributes to the continuous improvement of service.
